Diabetic Medical Plan Privacy Policy

The following Privacy Policy governs the online information collection practices of Diabetic Medical Plan (“we” or “us”). Specifically, it outlines the types of information that we gather about you while you are using the Diabetic Medical Plan website (the “Site”), and the ways in which we use and don’t use this information.

Please read this Privacy Policy carefully. By visiting and using the Site, you agree that your use of our Site, and any dispute over privacy, is governed by this Privacy Policy and our Terms and Conditions. Because the Web is an evolving medium, we may need to change our Privacy Policy at some point in the future, in which case we’ll post the changes to this Privacy Policy on this website and update the Effective Date of the policy to reflect the date of the changes. By continuing to use the Site after we post any such changes, you accept the Privacy Policy as modified.

This children’s privacy statement explains Diabetic Medical Plan’s practices with respect to the online collection and use of personal information from children under the age of thirteen, and provides important information regarding their rights under federal law with respect to such information.
  • This Site is not directed to children under the age of thirteen and we do NOT knowingly collect personally identifiable information from children under the age of thirteen as part of the Site. We screen users who wish to provide personal information in order to prevent users under the age of thirteen from providing such information. If we become aware that we have inadvertently received personally identifiable information from a user under the age of thirteen as part of the Site, we will delete such information from our records. If we change our practices in the future, we will obtain prior, verifiable parental consent before collecting any personally identifiable information from children under the age of thirteen as part of the Site.
  • Because we do not collect any personally identifiable information from children under the age of thirteen as part of the Site, we do NOT allow the participation of a child under thirteen in the Site’s online purpose.

privacy@DiabeticMedPlan.com or calling us toll-free at (888) XXX-XXXX during normal business hours. In the event that you do NOT update changes to your information, Diabetic Medical Plan will not be held responsible for the failure to provide supplies and services to you as agreed.

Disclosure for Discount Portion of the Plan:
  1. This plan is not a health insurance policy.
  2. This plan provides discounts at certain healthcare providers for medical services.
  3. This plan does not make payments directly to providers of medical services.
  4. The Plan member is obligated to pay all health care services but will receive a discount from those health care providers who have contracted with the Discount Medical Plan Organization.

This program and the program administrators have no liability for providing or guaranteeing service nor any liability for the quality of service rendered. ATTENTION MARYLAND RESIDENTS Some discounts under the Physician and Hospital Referral Plan benefit are not applicable in Maryland. Discounts are not available for all In-Patient Procedures and certain Out-Patient Procedures under Maryland law. Out-Patient Procedures at network hospitals such as laboratory and diagnostics services are eligible for the discount.
